Groves and other county hospitals officially join Guelph Wellington Ontario Health Team

Ontario health teams are being developed across the province as a way to deliver more connected health care from providers

Wellington County hospitals have officially joined what is now known as the Guelph Wellington Ontario Health Team (OHT). 

The Wellington Health Care Alliance announced in a press release Monday Groves Memorial Community Hospital (GMCH), Louise Marshall Hospital and Palmerston and District Hospital will be joining what was formerly called the Guelph and Area Ontario Health Team. 

These health teams are being developed across the province to provide a way of organizing and delivering more connected care from various providers. 

“Working together with our local partners will build a better healthcare experience for patients,” past GMCH board chair Jackie Ranahan explained in a press release. “Our team will work together to share information and coordinate with one another to make it easier for patients to receive the care they need from a single, coordinated team of providers.”

Rob Mungham and Susan Honeyman , patient advisors working with the OHT, said this will provide better population health in the region because of the patient-centered system. 

The three hospitals will be joining partners of the Guelph Wellington OHT including:

  • East Wellington Family Health Team
  • Guelph Family Health Team
  • Guelph Community Health Centre
  • Guelph General Hospital
  • Canadian Mental Health Association Waterloo-Wellington
  • The Elliott Community
  • St. Joseph’s Health Centre Guelph
  • Hospice Wellington
  • Home and Community Support Services Waterloo Wellington

“Initial planning efforts for the Guelph Wellington OHT will focus on palliative, mental health and addictions populations and enhancing patient care coordination within primary care teams,” the press release states.
