Provincial grant expands local paramedic remote patient monitoring program

GUELPH/WELLINGTON – The local paramedics will get to expand a program in part due to a grant from the Ministry of Health. 

The Guelph Wellington Ontario Health Team (GWOHT) announced in a Friday morning press release they received a grant worth nearly $145,000 from the Ontario Ministry of Health. 

This will be used to expand Community Paramedicine’s remote patient monitoring program, which supports older adults to live independently. Equipment is provided in client homes to monitor health indicators to allow them and health care providers to make informed decisions. 

A press release said the program seeks to keep individuals waiting for long-term care, frequently visit the hospital or lacking healthcare resources to remain safe in their homes.

“By increasing the reach of this program, we can improve the level of care in our community for those who need assistance,” said Leanne Swantko, deputy chief of GW Paramedic Service. 

The press release stated the initial phase of the program has reduced 911 calls, emergency department visits and hospital admissions.
