Empowered Seniors Speaker Series - Wednesday February 12, 2025.
There have been many changes from the Ontario Ministry of Health over the last few years. It can be confusing to figure out who did what when and what they do now. The Local Health Integration Network (LHIN) became Home and Community Care Support Services (HCCSS) in 2021 and now, the HCCSS has changed to Ontario Health atHome.
Ontario Health atHome is now a single organization dedicated to supportive home care services. Their mission is to help everyone be healthier at home through connected, accessible, patient-centred care, either in their own homes or via community care services. In February’s Empowered Seniors Speakers Series a representative from Ontario Health atHome will join us to share information about the services they provide. In this session, Ontario Health atHome - New Name, Same Exceptional Services, will share how they care for thousands of patients across the province every day.
During this seminar, you’ll learn:
- How patient care needs are assessed
- How you can remain safely at home with the support of health and other care professionals
- What happens after the return home from hospital and how to recover at home
- How Ontario Health atHome can help find a family doctor or nurse practitioner
- Where to find community services to support healthy, independent living
- Their role in Ontario’s long-term care home placement process
